I, (the "Participant"), hereby grant Massage Center and its representatives, agents, employees, and any affiliated entities (collectively, the "Center"), the irrevocable and unrestricted right to use, reproduce, and publish photographs and/or video recordings of me, taken by the Center or its representatives, for the purpose of promoting and marketing the Center's services through various channels, including but not limited to the Center's website, social media platforms, brochures, and other promotional materials.

I understand and agree that:

1. The images may be used by the Center indefinitely, without any limitation on the duration of usage.
2. The images may be used in any form, format, or medium, whether now known or hereafter created, including print, digital, electronic, or online platforms.
3. The images may be altered, edited, or modified as the Center deems appropriate, without further consent from me.
4. I will not receive any compensation, payment, or royalty for the use of the images, and I hereby waive any right to such compensation, payment, or royalty.
5. I have the right to revoke this permission at any time by providing written notice to the Center. Upon receipt of such notice, the Center will cease the use of the images in any new promotional materials and will make reasonable efforts to remove the images from existing materials, where feasible.

By signing below, I represent that I am at least 18 years old and have the legal capacity to enter into this agreement. If the Participant is under 18 years old, a parent or legal guardian must sign on their behalf.
